Over the year, I frequently gave my students formative assessments, so were scored and others were just to see which students were mastering the standard I was teaching and which weren't so that I could work in small groups with those not attaining the information. The ones that have been most useful are quiz index card assessments or dry erase board assessments. Four to five questions will typically get you what you are looking for on one standard.

Formative Assessment for Reading: Students create a Tagxedo on Tagxedo.com compiling all the main words from a particular story. Students pick the shape to put their words in that goes most with the main idea of the story. Students also pick the theme of colors that goes along with the story. This helps students focus in on the main pieces of the text they are reading and they enjoy making Tagxedos. Example:

Math Formative for factors: Compare and Contrast Map, also know as a double bubble map, to show common factors. You put one number in one circle and another number in another circle. As you think of the common factors you put the ones that are the same in between the two numbers and the ones that are different only connected to the bubble it belongs to. My students love this!

Formative Assessments are huge in Volusia County Public Schools. Many teachers are in search of quick formative assessment ideas. Currently I use formatives that range from a quick four questions on an index card, a couple problems on a dry erase board, or asking students a couple questions and they input them on their clickers for a specific standard.
